🩺 BSA Annual Health and Medical Record (AHMR)
As we prepare for upcoming campouts, summer camps, and other outdoor adventures, it's important to make sure that every Scout and adult leader has a current BSA Annual Health and Medical Record (AHMR) on file.
🩺 What’s Required?
The BSA Medical Form is divided into parts:
- Part A & B – Required for all participants. Includes general health info and emergency contacts.
- Part C – Required for events lasting 72 hours or more (like summer camp). This section must be completed and signed by a licensed medical professional after a physical exam.
📅 When Are They Due?
Please submit your completed forms to ensure we’re ready for our next major event. Medical forms must be updated annually, so even if you’ve submitted one before, it may need to be refreshed.
📥 Where to Get the Form

🔒 Why It’s Important
These forms help us:
- Ensure the safety and health of all participants
- Communicate effectively in emergencies
- Meet BSA’s national standards and camp requirements
📬 How to Submit
Please turn in printed forms to Troop Secretary Michelle Vonderau. Forms are kept confidential and only shared with those responsible for your child’s safety.
